Medical GUIDE Program

GUIDE Medicare Benefit Program is available for those living with dementia & their unpaid caregivers. This benefit of up to $2,563.00

seeks to provide support at every step of the disease progression. Those enrolled are assigned a Dubin Center Care Navigator who can provide support, connections to resources, and education. Enrollees’ caregivers are also eligible for a yearly stipend to help pay for respite services. The Dubin Center is an official GUIDE Partner Organization with Healthspan Partners of SWFL.

Eligibility Requirements: Traditional Medicare, a dementia-related illness diagnosis, not on hospice or PACE programs, living in assisted living, independent living, memory care, or at home (NOT skilled nursing)
